I understand that the minimum charge for Non-Destructive Digging (NDD) / Hydro Excavation is $250 per hour plus GST, plus an additional 12% temporary fuel surcharge. Charges are calculated from depot to depot and billed in 15-minute increments. If the mud dug up is not left on site, additional waste disposal charges will apply.
I agree that payment for these works will be made on the next business day after the invoice is received by email, with the invoice to be sent to , and that it will be sent from accounts@geelongcablelocations.com.au as a PDF attached to an email.
I acknowledge that, if the invoice is not paid as agreed, additional charges will be added to the invoice as per the Geelong Cable Locations Service Agreement. If the invoice remains unpaid, I, , will be personally liable, and legal action may be taken against me.

I am aware that this EFT payment authorisation applies only to works at the site address and/or site name entered above. Upon successful payment, this form and the copy of my driver licence will be deleted. For any future work that I wish to pay for via EFT, I will need to fill out a new form.
